Summary: The Waiver Case Manager supports individuals with developmental disabilities by connecting them to Indiana's state-funded waiver services. This role involves conducting assessments, developing individualized service plans, coordinating multidisciplinary care, and ensuring compliance with reputed company and state regulations. The Case Manager acts as a client reputed company, promoting independence and well-being while maintaining accurate documentation and contributing to quality improvement initiatives. Responsibilities: Assess clients’ needs, strengths, and goals to determine eligibility. reputed company and regularly review Individualized Service Plans (ISPs). Requirements

Knowledge of Indiana waiver programs (HCBS), reputed company regulations, and compliance standards. Proficiency with EHR/case management software. Excellent communication and organizational skills. Valid driver’s license, reliable transportation. Experience with person-centered and trauma-informed care. Ability to manage multiple caseloads, prioritize, and meet deadlines.
