Job Responsibilities The Health Coach (Cardiology) will function as a critical member of the Heart Failure Program and will work closely with the heart failure clinical team to provide patient education, guidance, and satisfaction, and ensure that our patients come first. The Health Coach will be responsible for guiding, educating, and bringing solutions to patients in need. • Provide patient education on chronic disease management, particularly educating them on their heart failure diagnosis and management.
• Provide motivational and clearly communicated educational components including heart health, diet, exercise, medication adherence.
• Establish short-term and long-term goals as part of shared decision-making with the patient and monitor patient reputed company throughout the program.

• Health coach certification from a program accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accrediting Bureau of Health Education Schools (ABHES). A bachelor’s degree in health sciences preferred.
• Experience in a medical office setting or healthcare environment. Experience in cardiology-reputed company coaching a plus.
• Experience with telehealth a plus.
• Strong interpersonal skills to communicate effectively and pleasantly with patients and other care team providers.
• Excellent customer service skills with the goal of achieving high quality customer satisfaction.
