Job Summary
As a Licensed Social Worker in our remote medical social work and case management team, you'll play a critical role in partnering with registered nurse case managers to assess, plan, and advocate for our members' benefit plans and health needs. You'll work collaboratively with our clinical staff to empower our members to make positive lifestyle and medical choices, and provide them with access to community resources for medical, financial, and behavioral health.
Responsibilities
* Research available options and resources for members with financial, transportation, safety, and housing needs
- Collaborate with registered nurse case managers to assess, plan, and advocate for members' benefit plans and health needs
- Empower members to make positive lifestyle and medical choices
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Utilize a collaborative process of assessment, planning, and advocacy to meet members' benefit plans and health needs
- Conduct telephonic or electronic exchanges with members to assess their needs and provide support
Requirements
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
- 2+ years of experience in medical social work or case management
- Masters Degree in Social Work

- Experience with electronic medical records and other healthcare software
