As a remote worker, you'll enjoy the flexibility of working from home, with virtual training and a hybrid schedule that includes Monday to Thursday, 8:30 am - 8:00 pm EST, and Friday, 8:30 am - 5:00 pm EST. As a Health Coach Consultant, you will be the driving force behind our mission to bring heart to every moment of our patients' health. You will work closely with patients and their care teams, utilizing your exceptional motivational interviewing skills to discern their health status and needs. With your strong clinical judgment, you will assist members in reducing risk factors and barriers, addressing complex health and social indicators that impact care planning and resolution of member issues.

Key Responsibilities:

• Utilize motivational interviewing skills to ensure maximum member engagement and discern their health status and health needs
• Apply clinical judgment to assist members in reducing risk factors and barriers, addressing complex health and social indicators
• Collaborate with clinical colleagues, supervisors, Medical Directors, and other programs to overcome barriers and meet goals and objectives
• Identify and escalate member needs appropriately, following set guidelines and protocols
• Actively reach out to members to collaborate and guide their care

Requirements:

• 5+ years of experience as a Registered Nurse
• 1+ year of nursing experience in patients with Diabetes, Cardiac Disease, ICU, CCU, Home Health, or Pulmonary Disease
• 1+ year of experience documenting electronically using keyboard and multiple computer screens
• Active and unrestricted RN licensure in the state of residence
• Associate's degree or diploma required, Bachelor's preferred

Preferred Qualifications:

• 1+ year of current or previous experience in Home Health and/or Case Management
• 1+ year of experience with Lexicomp
• 1+ year of experience with Motivational Interviewing
• Certified Diabetes Care and Education Specialist (CDCES)
• Bilingual in Spanish
