Your efforts will directly support transplant recipients in their journey to support their new kidney and live a healthy life.Key Responsibilities:Perform in-person Evaluation & Management (E&M) appointments for transplant recipientsReview labs and clinical data to make timely care plan adjustments in collaboration with supervising nephrologistsManage and adjust immunosuppressant therapies and chronic disease medicationsHandle prescription management, refills, and patient referralsMaintain a thorough and comprehensive EHR for each patient, including accurately recording the appropriate diagnosis codes and treatment codesSchedule & Work Environment:Full-Time, Monday–Friday. Hybrid Remote.
