Geriatric Social Worker (Hybrid/ Remote Part time Job description Responsibilities: - Conduct assessments and evaluations to determine clients' needs and develop appropriate treatment plans - Collaborate with other healthcare professionals to coordinate care and ensure holistic treatment - Assist clients in accessing community resources and support services - Maintain accurate and up-to-date documentation of client progress and treatment plans Experience: - Minimum of [1] years of experience as a Geriatric Social Worker or related field (Care Manager, Case Manager, Discharge Planner/ Coordinator etc) -BSW, MSW, Psychology or Human Service degree (any related field ) - Ability to navigate medical documentation systems and maintain confidentiality of client information - Experience working as a Discharge Planner in a nursing home or hospital is required Looking for someone with a positive attitude and an all-inclusive mindset.
