Analyst - Care Planner

We are looking for an experienced Analyst – Care Planner to join CVS Health to assess and analyze patient care plans and health risks. This role requires a proactive and detail-oriented individual to ensure the continuity and effectiveness of care plans. Successful candidates will be able to demonstrate strong problem-solving skills and have a passion for providing excellent patient care.To be... successful in this role, you should have a minimum of two years of experience as an Analyst – Care Planner or in a related role. You should also possess excellent communication, organizational, and analytical skills. Responsibilities: Analyze patient care plans and health risks to ensure continuity and effectiveness. Identify and assess patient health risks and develop corresponding care plans. Collaborate with healthcare teams to ensure optimal patient outcomes. Monitor patient care plans and present updates to healthcare teams. Maintain accurate records of patient care plans and health risks. Provide support and education to patients and caregivers on care plans. Stay up to date on latest healthcare trends and best practices. Address any issues or concerns with patient care plans. Utilize problem-solving skills to identify creative solutions to patient care plans. Ensure compliance with relevant healthcare regulations and guidelines.
