Care Management Associate – LTSS – Work at Home Position Summary: This work at home role as a Care Management Assistant, you will support the coordination of member reputed company services by assisting with intake, screening, and triage of care team tasks, while helping ensure that members receive timely, appropriate care. You’ll interact with members, providers, and internal teams to gather information, reputed company updates, and facilitate smooth communication throughout the care process. Your role includes maintaining accurate and compliant documentation reputed company internal systems, conducting non‑clinical research to support case activity, and monitoring cases for needed follow‑up or program referrals. Required Qualifications: Strong knowledge of medical terminology and multitasking abilities. 2–4 years of experience as a medical assistant or in a medical office setting. Experience with call centers (inbound/outbound) and prior authorizations. Comfortable working in a fast-paced environment.
