[Remote] Healthcare IT Project Manager, Population Health

They are seeking a Healthcare IT Project Manager to lead strategic initiatives in Medicare, Managed Care, and Population Health programs, driving complex healthcare technology projects from concept through implementation.
Responsibilities
- Provide end-to-end project leadership for multiple healthcare technology and operational initiatives, from project initiation through implementation, activation, and transition to operations
- Serve as the primary Project Manager supporting Medicare, Managed Care, and Population Health programs, reporting directly to the Program Manager
- Establish and maintain project governance, including project charters, schedules, forecasts, resource planning, risk management, and status reporting
- Collaborate with business, clinical, technical, and vendor stakeholders to ensure successful project execution, timely delivery, and alignment with organizational objectives
- Lead project team meetings, manage project artifacts, track milestones, issues, risks, and dependencies, and provide regular updates to leadership
- Forecast project resources, monitor financial performance, support capital planning activities, and ensure efficient allocation of resources across initiatives
- Drive implementation of tools and systems supporting longitudinal patient tracking, care coordination, health data integration, and value-based care initiatives
- Proactively identify barriers, escalate risks when appropriate, and ensure open communication among all project participants to achieve successful outcomes
Skills
- 5+ years of Project Management experience leading complex healthcare IT, clinical systems, EHR, or population health initiatives
- Proven experience managing multiple projects simultaneously through the full project lifecycle, from planning and execution through implementation and operational transition
- Strong experience delivering EHR-related projects within provider organizations; Cerner experience preferred, Athena experience considered
- Experience working with product development teams, Agile methodologies, or healthcare technology delivery teams
- Knowledge of care coordination, Health Information Exchange (HIE), interoperability platforms, population health, or value-based care programs
- Experience with project financial management, resource forecasting, budget tracking, capital resource allocation, and project governance processes
- Experience utilizing ServiceNow and other project management tools in an enterprise healthcare environment
- Bachelor's degree in Healthcare Administration, Information Technology, Business Administration, Project Management, or related field
- Clinical background or healthcare operations experience preferred
- Project Management Professional (PMP) certification is preferred
- Prior experience supporting large integrated delivery networks or enterprise health systems
