The successful candidate will be responsible for coordinating and scheduling patient admissions, visits, and services, ensuring seamless delivery of care to our patients and their families. Availability: Full-Time | Open to Remote Work Job Responsibilities: Schedule patient admissions with Admission nurse; F2F visits with Nurse Practitioner or Hospice Physician Manage and coordinate schedules for interdisciplinary team visits (IDG), including physicians, nurses, social workers, spiritual care providers and ancillary staff. Communicate effectively with patients, families, and healthcare professionals to ensure smooth transitions and continuity of care. Maintain accurate and up-to-date records of patient schedules, visits, and services. Collaborate with the admissions/intake team to ensure timely and efficient patient intake processes. Troubleshoot scheduling conflicts and resolve issues promptly. Provide administrative support to the admissions/intake team as needed. Be on top of all PTO or time off request by staff and inform team Inform facilities, families of staff schedules, any changes via email, calls or faxes or a combination of. Assist with plotting orientation schedule for new hires, any scheduled CPR classes, any required or highly encouraged meetings, team meeting schedules Other projects as directed by a manager Minimum Qualifications: High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred. 1-2 years of experience in healthcare scheduling or a related field. Excellent communication and organizational skills. Ability to work in a fast-paced environment and prioritize multiple tasks. Compassionate and empathetic approach to working with patients and families. Proficiency in electronic medical records (EMRs) and scheduling software. Familiar with the geographical location of different counties we serve: San Francisco, San Mateo, Alameda and Contra Costa. Preferred Qualifications: Experience working in hospice or palliative care. Certification in medical scheduling or a related field. Knowledge of healthcare regulations and compliance. Apply To This Job
