Senior Clinical Social Worker - Remote Hospice Services
Role Overview People Concern Counseling, LLC is seeking a dedicated Senior Clinical Social Worker to join our compassionate team in delivering high-quality care within a remote hospice setting. This full-time position offers the flexibility of working from home while making a meaningful impact in the lives of patients and their families during critical times.
Key Responsibilities
- Provide comprehensive psychosocial assessments and develop individualized care plans for patients.
- Deliver therapeutic interventions and support to patients and their families, focusing on emotional and social needs.
- Collaborate with an interdisciplinary team to ensure holistic care and seamless service delivery.
- Facilitate support groups and educational workshops for families and caregivers.
- Maintain accurate and timely documentation in compliance with organizational and regulatory standards.
- Stay informed about community resources and facilitate referrals as needed.
Required Qualifications
- Master's degree in Social Work (MSW) from an accredited institution.
- Current and valid state licensure as a Clinical Social Worker.
- Minimum of 3 years of clinical experience, preferably in hospice or palliative care.
- Strong interpersonal skills and the ability to work effectively with diverse populations.
- Excellent communication skills, both written and verbal.
