Join our team as a Remote Case Manager RN and play a critical role in guiding members toward better health outcomes through personalized care coordination. What You’ll Do Conduct comprehensive clinical assessments and develop individualized care plans Coordinate inpatient reviews and discharge planning Advocate for members and connect them with appropriate services and resources Monitor care plans and adjust strategies to ensure optimal, cost-effective outcomes Use motivational interviewing and coaching to support behavior change and engagement Identify quality of care issues and contribute to program improvements Deliver exceptional member service in a remote environment What You Bring - REQUIRED Active, unrestricted RN license (with ability to obtain multi-state licensure paid for by the company) 3+ years of acute care RN experience Strong clinical judgment and decision-making skills Excellent communication, organization, and multitasking abilities Proficiency in Microsoft Office tools

Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $54,095.00 - $116,760.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
