Job Overview - Registered Nurse (Hybrid): Join our team as a Registered Nurse, where you'll play a crucial role in evaluating patients' health status and developing personalized long-term care service plans. This position requires a blend of clinical expertise, technological proficiency, and excellent communication skills to ensure the highest quality of care for our clients. Location: New York, openings available in all boroughs (2 days telehealth, 3 days on-site) Compensation: $50-$55/hr Responsibilities of the Registered Nurse:
- Conduct initial in-home pre-evaluations of patients to assess health status, care needs, and preferences, guiding the development of individualized service plans.
- Review relevant medical documentation and health referral forms to inform evaluation processes.
- Enter evaluation data into electronic forms and transmit as required, ensuring accuracy and completeness.
- Document any concerns or issues arising during the evaluation process, emphasizing continuity of care.
- Discuss healthcare options, support needs, and service vendor options with consumers, facilitating informed decision-making.
- Foster communication and collaboration with other team members to improve care coordination and service delivery.
- Perform additional duties as assigned by the Regional Nurse Manager or project manager.
Qualifications for the Registered Nurse:
- Minimum of two (2) years of experience in a home setting. (Certified Home Health Agencies (CHHAs), Licensed Home Care Service Agencies (LHCSAs), or Managed Long-Term Care (MLTC)).
- Strong knowledge of general professional nursing principles and state/agency regulations.
- Excellent organizational, interpersonal, written, and verbal communication skills.
- Ability to work effectively with multi-generational consumers.
- Comfortable working in a fast-paced, deadline-oriented environment.
