Responsibilities
- Provide personal care and assist with ADL's according to the POC.
- Provide a safe environment for patients.
- Safely ambulate/transfer patient as appropriate.
- Assist with passive and active ROM exercises as directed by the nurse.
- Prepare nutritional snacks as appropriate for the patient.
- Under the supervision of the nurse, assist patient with pre-poured medications that are ordinarily self-administered and that are prescribed by a physician under the supervision of the nurse (Home Care only).
- Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
- Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
- Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
- Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
- Attend and actively contribute during patient care conference.
- Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
- Attend in-services to obtain 3 education hours per quarter.
- Participate in the on-call rotation as scheduled (Home Care only).
- Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.).
- Demonstrate positive customer-service responsiveness according to agency standards.
- Flexible in meeting patient/family needs that require variable shifts and weekends.
- Participate in chart review quality improvement activities and other meetings as assigned.
- Participate in coordination of and communication with volunteer staff.
- Provide basic care-giving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens.
- Maintain Transmission Based Precaution.
- Complete annual competency requirements.
Requirements
- Must be able to read and write English proficiently.
- Must be a state-tested nursing assistant in good standing.
- Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.
- Compliance to Annual Flu Shot Policy or ability to provide exemption documentation.
- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed.
- Provide an active driver’s license.
- Ability to provide proof of eligibility to work in the United States.
- High level of integrity, ethics and professionalism.
