Pay: $250,000 - $275,000 base + bonus + equity Schedule: Standard weekday leadership hours Help build scalable clinical programs focused on care gap closure, risk adjustment, and better member outcomes. This role works closely with NPs, operations, and care teams to improve workflows across home-based, virtual, and provider-connected care. What you’ll do Lead value-based care workflows across multiple care settings Manage and support NP clinical performance Improve care gap closure and RAF accuracy Build new clinical programs and operating processes Standardize documentation and clinical quality workflows Partner with operations and care delivery leaders Help scale population health initiatives Must-Haves MD or DO required 3+ years in value-based care Experience managing NPs or clinical teams Strong knowledge of care gap programs Strong knowledge of risk adjustment programs Experience building clinical workflows Comfortable in fast-moving operational environments
