Experienced Licensed Social Worker – Remote Medical Social Work and Case Management

As an Experienced Licensed Social Worker, you'll play a critical role in partnering with our registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet our members' benefit plan and/or health needs.

Job Summary

We're seeking a highly skilled and compassionate Licensed Social Worker to join our remote team. As a key member of our medical social work and case management team, you'll work closely with our registered nurse case managers to provide exceptional care to our members. Your expertise will be invaluable in researching available options and resources for members with financial, transportation, safety, and housing needs. This is a part-time, remote role that requires a strong commitment to delivering high-quality care and support to our members.

Responsibilities

As an Experienced Licensed Social Worker, your key responsibilities will include:

• Partnering with registered nurse case managers to assess, plan, and advocate for members' benefit plan and/or health needs
• Researching available options and resources for members with financial, transportation, safety, and housing needs
• Empowering members to make positive lifestyle and medical choices
• Collaborating with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Providing telephonic or electronic support to members across the entire lifecycle
• Utilizing a collaborative process to develop and implement care plans that meet members' unique needs

Qualifications

To be successful in this role, you'll need:

• An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
• At least 2 years of experience in medical social work or case management
• A strong understanding of healthcare systems, policies, and procedures
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong problem-solving and critical thinking skills
• Proficiency in electronic medical records and other relevant software systems
