Experienced Licensed Social Worker – Remote Medical Case Management

Job Summary
We're seeking an experienced Licensed Social Worker to join our remote medical case management team. As a key member of our team, you'll work collaboratively with a registered nurse case manager to assess, plan, and advocate for members' benefit plans and health needs. Your expertise will be invaluable in researching available options and resources for members with financial, transportation, safety, and housing needs. This role requires a strong understanding of medical social work and case management principles, as well as excellent communication and interpersonal skills.
Responsibilities
* Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs
- Empower members to make positive lifestyle and medical choices
- Work collaboratively with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members to assess their needs and provide support
- Develop and implement individualized plans to address members' needs and goals
- Maintain accurate and up-to-date records of member interactions and progress
- Participate in ongoing education and training to stay current on best practices in medical social work and case management
Qualifications
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence
- 2+ years of experience in medical social work or case management
- Master's degree in Social Work or related field
- Strong understanding of medical social work and case management principles
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively as part of a team
- Strong problem-solving and critical thinking skills
- Ability to maintain confidentiality and handle sensitive information with discretion
