Job Summary
We're seeking an Experienced Licensed Social Worker to join our remote team. As a licensed Masters Social Worker, you'll work with a registered nurse case manager to research available options and resources for members with financial, transportation, safety, and housing needs. This role requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical social work or case management.
Responsibilities
* Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs
- Empower members to make positive lifestyle and medical choices
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members
- Collaborate with other departments to ensure seamless care coordination
Requirements
* Active and unrestricted LSW - Licensed Social Worker, LMSW - Licensed Masters Social Worker, LCSW - Licensed Clinical Social Worker, or LISW - Licensed Independent Social Worker in state of residence
- 2+ years of experience in medical social work or case management
- Masters Degree in Social Work

Work Environment
* Remote work from home role
- Must be able to work anywhere in the U.S.
- Working hours will be Monday-Friday 8am-4:30pm Pacific Standard Time
- No weekends or holidays will be required
