Job Summary

We're seeking a highly skilled and experienced Licensed Social Worker to join our remote team. As a Medical Case Manager and Advocate, you'll work with a registered nurse case manager to assess, plan, and advocate for members with financial, transportation, safety, and housing needs. This is a part-time remote role, requiring Monday-Friday 8am-4:30pm Pacific Standard Time. No weekends or holidays will be required.

Responsibilities

* Research available options and resources for members with financial, transportation, safety, and housing needs

• Collaborate with registered nurse case managers to assess, plan, and advocate for members
• Empower members to make positive lifestyle and medical choices
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
• All member contact will be done telephonically or via electronic exchange

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence

• 2+ years of experience in medical social work or case management
• Masters Degree in Social Work
