Overview
The Virtual Care Navigator will be a crucial part of our healthcare team, ensuring patients receive consistent and effective care while undergoing treatment remotely. This role involves monitoring patient progress, providing support and education, and coordinating with healthcare providers to optimize treatment outcomes. Responsibilities
- Patient Monitoring and Support: Continuously monitor patient data and treatment adherence through remote monitoring systems; track and document patient progress, symptoms, and treatment side effects; maintain regular communication with patients to provide support, answer questions, and address concerns; facilitate virtual check-ins to ensure patients understand their treatment plans and adhere to prescribed therapies.
- Coordination with Healthcare Providers: Act as an advocate for patients, addressing concerns or barriers to accessing virtual care services and adhering to treatment; coordinate adjustments to treatment plans based on patient feedback and monitoring data by relaying important patient information to the primary Therapist.
- Technical Assistance: Assist patients in setting up and using remote monitoring devices and applications, ensuring they are comfortable and confident in using the technology; troubleshoot technical issues and provide guidance on using health management software.
- Education and Resource Provision: Educate patients and families about conditions, treatment plans, and the use of remote monitoring tools; provide resources and materials to support patient self-management and understanding of health conditions.
- Data Management and Reporting: Ensure accurate and timely documentation of patient interactions and treatment outcomes; prepare regular reports for healthcare providers and participate in case review meetings.
Qualifications
Minimum Education
- Required: Associate’s degree in Physical Therapy Assistant
Preferred:
- Bachelor degree in Physical Therapy Assistant
Minimum Experience
- Required: 3-5 years of experience as a licensed PTA
Preferred:
- 1 or more years of experience in telerehab
Technical Skills
- Proficient in using remote patient monitoring systems and electronic health record (EHR) systems
- Strong computer skills, including familiarity with telehealth platforms and health management software
Interpersonal Skills
- Excellent communication and interpersonal skills, with the ability to convey information clearly and empathetically
- Strong organizational skills and attention to detail
- Ability to work independently and collaboratively in a remote environment
Professional Skills
- Strong problem-solving abilities and a proactive approach to patient care
- Knowledge of healthcare policies, remote monitoring regulations, and patient privacy laws (e.g., HIPAA)
Licenses and Certifications
- Required: Licensed Physical Therapist Assistant or Occupational Therapy Assistant in home state, and eligible for licensure in multiple states as needed, defined by ATI
- Preferred: Compact license eligible
