Experienced Licensed Social Worker – Remote Medical Case Management

We're seeking an experienced Licensed Social Worker to join our remote medical case management team, working collaboratively with registered nurses to empower our patients to make positive lifestyle and medical choices.

Job Summary:

As a Licensed Social Worker at arenaflex, you'll play a vital role in assessing, planning, and advocating for our patients' benefit plans and health needs. You'll research available options and resources to address financial, transportation, safety, and housing needs, working with a diverse population across the entire lifecycle. This is a remote work-from-home opportunity, requiring a strong ability to work independently and collaboratively as part of a team.

Responsibilities:

* Collaborate with registered nurse case managers to assess patients' benefit plans and health needs

• Research and identify available options and resources to address financial, transportation, safety, and housing needs
• Empower patients to make positive lifestyle and medical choices through education and advocacy
• Work with other clinical staff to assist patients with community resources for medical, financial, and behavioral health
• Utilize a collaborative process of assessment, planning, and advocacy to meet patients' needs
• Conduct telephonic or electronic exchanges with patients to provide support and guidance
• Maintain accurate and up-to-date records of patient interactions and progress

Requirements:

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence

• 2+ years of experience in medical social work or case management
• Masters Degree in Social Work or related field
• Strong communication and interpersonal skills
• Ability to work independently and collaboratively as part of a team
• Strong problem-solving and critical thinking skills
• Proficiency in electronic medical records and other relevant software systems

Preferred Qualifications:

* Experience working with diverse populations and age groups

• Knowledge of community resources and services for medical, financial, and behavioral health
• Certification in case management or social work
• Experience working in a healthcare setting or related field

Compensation and Benefits:

* Competitive salary range: $46,597.20 - $101,228.40
