Chronic Care Management Care Manager (LPN/RN)
Company overview: Lumina Care is focused on unifying, coordinating, and managing care for geriatric patients in nursing facilities and at home. We offer a range of services to improve health outcomes and quality of life for patients, including afterhours telehealth, transitional care, chronic care, remote patient monitoring, behavioral health and telehealth psychiatry. Position Overview: We are seeking a dedicated and experienced Care Manager (LPN/RN) to join our team. This remote role involves creating comprehensive care plans for patients in long-term care settings, conducting monthly chart reviews, and addressing care gaps. Key Responsibilities:
- Conduct monthly chart reviews to assess patient medical history, current care plan and identify any opportunities in care delivery to improve the management of their conditions.
- Update patient information to ensure accurate and up-to-date records.
- Active participant in the care team to review, augment and update care plans to be patient centered following patient goals of care and advanced care planning.
- Identify and collaborate to identify care opportunities to improve health outcomes and enhance the quality of life for patients.
- Collaborate with Team Leads and RN Clinical Managers for clinical growth and development.
- Be innovative in approach and utilize clinical skills while being a team player.
- Maintaining compliance with all healthcare regulations and company policies while delivering exceptional service to our patients.
- Effectively promote patient centered care by performing comprehensive chart reviews, identifying care improvement opportunities, and documenting findings with clarity and accuracy to support improved outcomes.
- Meets established performance metrics and quality benchmark scores.
- Support the Lumina Care mission to solve the problems in care delivery that slow down collaboration, coordination, and communication—causing care to be more complicated than necessary.
Qualifications:
- Licensure: Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current and valid certification.
- Experience: Minimum of one year of experience in long-term care settings.
- Skills: Strong organizational and communication skills, proficiency in remote care management tools, and the ability to work independently.
Preferred Qualifications:
- Experience with telemedicine platforms and remote care coordination.
- Familiarity with electronic health record (EHR) systems.
- Strong problem-solving skills and the ability to handle complex care scenarios.
Requirements:
- A laptop (Chromebooks are not compatible with our required tools and systems).
- An external monitor for efficient multitasking and productivity.
- Reliable internet connection.
- A quiet and dedicated workspace.
Pay: $27.00 - $35.00 per hour Benefits:
