Job Summary

As a Remote Social Worker, you'll play a vital role in our healthcare team, working collaboratively with a registered nurse case manager to assess, plan, and advocate for our members' benefit plan and health needs. Your expertise will be essential in researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a diverse population across the entire lifecycle, utilizing telephonic and electronic communication to empower members to make informed decisions about their health and well-being.

Responsibilities

* Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and health needs

• Research and identify available options and resources for members with financial, transportation, safety, and housing needs
• Develop and implement personalized plans to address members' complex needs
• Provide education and support to members to promote positive lifestyle and medical choices
• Work closely with other clinical staff to ensure seamless coordination of care and community resources
• Utilize telephonic and electronic communication to engage with members and provide exceptional customer service
• Maintain accurate and up-to-date records of member interactions and interventions

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence

• 2+ years of experience in medical social work or case management
• Master's degree in Social Work (MSW) or related field

• A base salary range of $46,597.20 - $101,228.40
