Experienced Licensed Social Worker – Remote Medical Case Management

Job Summary

We're seeking an experienced licensed social worker to join our remote medical case management team. As a social worker, you'll partner with a registered nurse case manager to assess, plan, and advocate for our patients' benefit plans and health needs. Your primary responsibility will be to research available options and resources for patients with financial, transportation, safety, and housing needs. This role requires a collaborative approach, working with other clinical staff to assist patients with community resources for medical, financial, and behavioral health.

Responsibilities

* Research and identify available options and resources for patients with financial, transportation, safety, and housing needs

• Collaborate with registered nurse case managers to assess, plan, and advocate for patients' benefit plans and health needs
• Develop and implement personalized care plans that address patients' unique needs and circumstances
• Provide education and support to patients and their families to empower them to make positive lifestyle and medical choices
• Work closely with other clinical staff to ensure seamless coordination of care and effective communication with patients and their families
• Utilize telephonic and electronic communication to engage with patients and their families
• Maintain accurate and up-to-date records of patient interactions and care plans

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence

• 2+ years of experience in medical social work or case management
• Master's degree in social work or a related field
• Strong communication and interpersonal skills
• Ability to work independently and collaboratively as part of a team
• Proficiency in electronic medical records and other healthcare software systems

Preferred Qualifications

* Experience working with diverse patient populations, including those with complex medical and social needs

• Knowledge of community resources and services that support patients' medical, financial, and behavioral health needs
• Familiarity with case management principles and practices
• Experience with telephonic and electronic communication with patients and their families
