Job Summary

We're seeking an experienced Social Worker to partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. As a remote Social Worker, you'll work with a diverse population, spanning the entire lifecycle, to research available options and resources for members with financial, transportation, safety, and housing needs. Your primary focus will be on empowering members to make positive lifestyle and medical choices, while working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.

Responsibilities

* Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs

• Research available options and resources for members with financial, transportation, safety, and housing needs
• Empower members to make positive lifestyle and medical choices through education and support
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Utilize a collaborative process to develop and implement individualized plans to meet members' needs
• Maintain accurate and up-to-date records of member interactions and progress
• Participate in ongoing education and training to stay current on best practices and industry developments

Qualifications

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence

• 2+ years of experience in medical social work or case management
• Masters Degree in Social Work or related field

• Strong knowledge of community resources and services, with the ability to research and connect members with relevant resources
• Ability to maintain accurate and up-to-date records, with a strong focus on data-driven decision-making

* Competitive salary range: $46,597.20 - $101,228.40
