Position: Licensed Vocational Nurse (LVN) - Community Health (CalAIM ECM Program)
Overview
At Pacific Health Group, we are dedicated to improving health outcomes by addressing social determinants of health and coordinating comprehensive community‑based services, particularly through our CalAIM programs. As a Licensed Vocational Nurse (LVN) on the Community Health team, you will deliver essential nursing services to underserved populations and support clients in their overall mental and physical health. Job Type: Full‑time Total to Hire: 2 Pay: $30.00 – $35.00 per hour Expected Hours: Monday through Friday, 8:30 AM to 5:00 PM (40 hours per week) Work Location: Hybrid in Santa Clara County, California
Responsibilities
- Provide direct nursing care to clients in community settings
- Assist in the assessment and evaluation of clients' health needs
- Develop and implement individualized care plans in collaboration with the healthcare team
- Monitor and document clients' health status and progress
- Educate clients and their families on healthcare, wellness, and preventive measures
- Coordinate with multidisciplinary teams to ensure a holistic approach to care
- Support clients in accessing mental health and social support services
- Conduct community‑based outreach across multiple counties to identify, engage, and enroll eligible members into ECM, Community Supports, and Behavioral Health programs
- Build and maintain partnerships to expand service access in high‑need communities
Requirements
- Residency in Santa Clara County
- Current and valid LVN license in the state of practice
- Minimum of one year experience in a healthcare or community health setting
- Strong understanding of mental health care principles and practices
- Excellent communication and interpersonal skills
- Ability to work effectively in a team‑oriented environment
- Compassionate and patient‑centered approach to care
- Willingness to travel within the community as needed
- Valid California Driver's License (Class C minimum) and possession of a personal, operable vehicle with current liability insurance meeting California’s minimum legal requirements.
- Background check clearance of a Motor Vehicle Report (MVR) prior to employment
