Job Summary

We are seeking an experienced Healthcare Customer Service Representative – Population Health Navigator to join our team for a 4-month contract, with the possibility of extension. As a key member of our customer service team, you will be responsible for conducting telephonic outreach to Cigna Healthcare Medicare customers, educating them on plan benefits, and addressing gaps in care. You will also coordinate with internal teams to ensure timely hand-offs to appropriate care teams, document all interactions for regulatory review, and assist with scheduling medical appointments and connecting customers to case management and community resources.

Responsibilities

As a Healthcare Customer Service Representative – Population Health Navigator, your key responsibilities will include:

  • Conducting telephonic outreach to Cigna Healthcare Medicare customers to complete health screenings
  • Educating customers on plan benefits and addressing gaps in care
  • Coordinating with internal teams to ensure timely hand-offs to appropriate care teams
  • Documenting all interactions for regulatory review
  • Assisting with scheduling medical appointments and connecting customers to case management and community resources
  • Escalating customer concerns or issues appropriately

Requirements

To be successful in this role, you will need to possess the following skills and qualifications:

  • Strong written and verbal communication skills with an empathetic attitude
  • Proficiency in navigating computer applications, including Microsoft Outlook, Excel, Word, and dual-monitor setups
  • Ability to handle multiple tasks, set priorities, and develop action plans with attention to detail
  • Knowledge of healthcare and regulatory requirements with an emphasis on Medicare
  • Experience in managed care, health services, or population management is preferred

Preferred Qualifications

While not required, the following qualifications are highly desirable:

  • 1+ years in managed care or a related healthcare field
  • 1+ years involving telephone contact and process management

Education

A high school diploma is required, with a college degree preferred.

Compensation and Benefits
