We are seeking a compassionate and resourceful Social Worker to join our remote care team. This role is ideal for someone who thrives in a collaborative healthcare environment and is passionate about helping patients navigate medical and social challenges. Position Overview The Social Worker will provide remote psychosocial support, care coordination, and resource navigation for patients enrolled in chronic care management programs. You will serve as an advocate and problem-solver, helping patients overcome barriers that impact their health and well-being. This position requires strong communication skills, clinical judgment, and the ability to work independently while maintaining high standards of professionalism and documentation.
Key Responsibilities
- Conduct comprehensive psychosocial assessments via phone or video
- Develop individualized care plans that address medical, emotional, and social needs
- Connect patients and families to community resources, financial assistance programs, and healthcare services
- Provide emotional support, short-term counseling, and crisis intervention as appropriate
- Assist patients in navigating insurance, transportation, housing, and food insecurity challenges
- Collaborate closely with nurses, care managers, and primary care providers
- Document all patient interactions accurately and timely in the EMR
- Maintain compliance with HIPAA and all organizational policies
- Stay informed on local and national social service resources and eligibility requirements
Qualifications
- Bachelor’s or Master’s Degree in Social Work (BSW or MSW) or related field
- Previous experience in healthcare, chronic care management, or case management preferred
- Strong clinical assessment and communication skills
- Comfortable working independently in a fully remote environment
- Proficiency with EMR systems, video conferencing platforms, and basic office technology
